REBREED/FUTURITY TRANSPORTED COOLED SEMEN AGREEMENT

I,
_________________________________________________ (Mare Owner/Leasee) have agreed to breed
________________________________________________________________ (Name and Registration # of mare) to MACS GOOD N PLENTY in the 2011
breeding season and hereby request that cooled semen be transported for
said breeding under the following terms and conditions:
1.
Please circle the appropriate breeding:
a. REBREED: All shipments of semen
will have a $280/shipment chute (collection/handling/shipping) fee which
includes overnight standard shipping to mare owner but not return cost of
container (shipping extra to Canada).
b. FUTURITY: All shipments of semen
will be $280/shipment which includes overnight standard shipping to mare
owner but not return cost of container (shipping extra to Canada).
2. If the mare does
not settle after three shipments of semen Stallion Management will contact
the Stallion Owner to discuss subsequent shipments.
3. The chute fee must be paid prior to semen
shipment. ISU LVMC accepts Visa,
Mastercard and Discover and personal check if paid in advance.
4. A credit card number must be given to ISU LVMC prior to semen shipment to
serve as a shipping container deposit. No charges will be made to the card
unless the container is not returned within the specified time period.
5. The shipping
container must be returned to ISU
LVMC within 4 business days or the Mare
Owner’s credit card will be charged $30 for an Equine Express container or
$300 for an Equitainer.
6. The breeding must
be done under direct supervision of a licensed veterinarian and have a
regular and timely teasing program. The mares must be examined by
ultrasound to assess breeding status and timely insemination.
7. The Mare Owner or
representative must notify ISU LVMC when the mare comes into heat.
8. The Mare Owner or
representative must give ISU LVMC a minimum of 24 hours notice when semen is
needed.
9. Semen will be
collected on Monday, Wednesday and Friday. ISU LVMC needs to be notified no later than 10 AM on collection
days of any semen requests for that day.
Semen requests will be filled in the order they are received and
Stallion Management will dictate fulfillment of semen requests.
10. ISU LVMC cannot be held liable for semen
once the shipment leaves the ISU LVMC facility.

hereby become a part of the Stallion Service Agreement between the Mare
Owner, Stallion Management and Conifer Quarter Horses (Stallion Owner)
